The Jefferson Medical Home Academic Center (Jeff MHAC) in the Department of Family and Community Medicine is the Center for Patient Centered Medical Home (PCMH) Research, Training and Dissemination, funded through HRSA (Health Resources and Services Administration). PCMH is a distinctive relationship based program that aims to engage both physicians and patients in an effort to strengthen the doctor-patient relationship and better coordinate care to achieve ONE care delivery system. Jefferson Family Medicine Associates (JFMA) has been recognized by the NCQA as a Level 3 PCMH, which means it has scored 75-100 overall points and 10/10 “must pass” requirements.
The PCMH model is an avenue to cater multifaceted primary care for children, youth and adults. It accelerates the partnerships developed between patients, their families and their primary care physicians.
