Implementing Decision Support Tools to Enhance Care for Older Adults

This Forum presentation will explore barriers to discharge referral decision making and provide an overview of the implementation of decision support tools into discharge planning workflow. The impact of discharge referral decision support on readmissions will also be discussed.
Kathryn H. Bowles, PhD, RN, FAAN, is Professor and Ralston House Endowed Chair for Gerontological Nursing and Associate Director of the New Courtland Center for Transitions and Health at the University of Pennsylvania School of Nursing. She is also the Beatrice Renfield Visiting Scholar for the Visiting Nurse Service of New York; a Senior Fellow in the Leonard Davis Institute; a faculty member in the Ackoff Center for Advancement of Systems Approaches; and Director of the Health Informatics Minor.
