Lynda Thomson is a clinical pharmacist specializing in antithrombotic management and transition of care services in the departments of Pharmacy and Medicine at the Jefferson Vascular Center, Thomas Jefferson University Hospital (TJUH). She received her BS and PharmD degrees from the Philadelphia College of Pharmacy and Science, completed a residency in Pharmacokinetics at TJUH and traineeships in phlebotomy, anticoagulation, hyperlipidemia and diabetes management. Lynda has practiced in the pharmacotherapeutic areas of infectious diseases, internal and vascular medicine.  She is a board certified anticoagulation provider, a licensed immunizing pharmacist and oversees formulary maintenance and antithrombotic policies at TJUH.   She serves on the Pharmacy Invitational Conference on Antithrombotic Therapy Steering Committee and works closely with the Society of Hospital Medicine to develop a national transition of care model for patients with acute VTED. She has trained pharmacists in antithrombotic management via ASHP’s Antithrombotic Preceptorship Program and is the recipient of ASHP’s Best Practices Award in 2010.  She was also the recipient of the Joe E Smith Award from the Pennsylvania Society of Health System Pharmacists in 2018.
