Student Health Insurance Plan

Every fall semester, students who are required to have health insurance that meets the University’s requirements must enroll or waive coverage on the University Health Plans site by the enrollment/waiver deadline.

The Student Health Center — available to all full-time students enrolled in courses at the East Falls campus — is dedicated to providing confidential, comprehensive, and accessible health care for all students.

Student Health Record Form

Thomas Jefferson University requires that you have a completed health history form on file in the Office of Health Services by the time you register for classes.

Without a completed health history, you may not be permitted to register for classes. Additionally, the Director of Health Services may require you to undergo a complete physical and immunization procedure if you cannot produce the form on or before registration.

The health form must be completed by a physician. Please send the completed health form to the university as soon as possible so your registration will not be delayed. 

Do not email the form; it will not be accepted. Please mail it by post or bring it with you to hand in upon arrival.
